About Us

Community Care Navigators, Inc. is a Florida nonprofit corporation and IRS-recognized 501(c)(3) public charity based in West Palm Beach. We give Palm Beach County residents free, one-on-one help finding and applying for the public benefits they may be eligible for.

Our mission

To help Palm Beach County residents find, understand, and apply for public benefits — such as SNAP, Medicaid, SSI, LIHEAP, Medicare Savings Programs, and housing assistance — free of charge.

Why we exist

Benefit programs can be hard to understand. Forms are long, rules change, and many people do not know what help is available. We provide one-on-one guidance so no one has to do it alone.

Our model

Our help is provided by trained staff. Their work is funded by grants and charitable support, so our services are free to the people we serve.
